We can help you find a doctor that's right for you. 
  • How do I find a doctor or location?
    To find a doctor or location, click the Find a Doctor button at the top of the webpage. From there, you will be able to see a list of in-network providers and locations. You will also have the option to filter your search results based on location, specialty, accepting new patients, language, gender and more. Or you can call us directly at 833-600-1311 for help finding the care that’s right for you.
  • What are network doctors?
    Network doctors are a list of healthcare professionals who are contracted with US Health and Life Insurance Company, the insurer that offers Ascension Personalized Care, and provide medical care to our members.
  • How do I get care from out-of-network doctors?

    You may visit a doctor outside of the network. However, because the doctor is not within the Ascension Personalized Care network, the amount billed for services will be higher. If you prefer to be seen by an out-of-network provider, you will be responsible for paying the difference between the amount billed and your plan's approved amount.
  • What is a PCP and how do I choose one?
    A PCP is a primary care provider. A PCP can be a doctor, nurse practitioner, clinical nurse specialist or physician assistant. These providers help patients with a wide range of healthcare services. To choose a PCP, click Find a Doctor and you will be able to browse doctors in your area that are accepting new patients.
  • How do I know where to go for care?
    Ascension Personalized Care offers many options to choose from including doctors offices, urgent care, emergency rooms and virtual or online care. Click the Find a Doctor button at the top of the webpage to find a doctor or location. You will also have the option to filter these results based on location, specialty, accepting new patients, language, gender and more. You can also call 833-600-1311 and one of our customer service agents will be happy to assist you in finding the right doctor.
  • Is telehealth or virtual care an option?

    Yes! We encourage all of our members to use Ascension Online Care for virtual urgent care visits. You can speak with a doctor 24/7, anytime, anywhere for your urgent care. Our virtual urgent care option is for minor illnesses such as fever, cold, flu, seasonal allergies, urinary tract infections, eye infections, and more. These visits are as secure and confidential as a face-to-face visit and protected by the same patient policy rules. Just visit your app store and look for Ascension Online Care.

    Your doctor may also offer virtual visits. Call your doctor to schedule an appointment and ask if a virtual visit is an option for your care.
  • How does telehealth or virtual care work?

    Virtual urgent care can be a helpful option when you need more immediate care. You can see a doctor 24/7 from anywhere. Just visit the app store on your phone and download Ascension Online Care. You will be prompted to create an account.

    Your doctor may also offer virtual visits. Call your doctor to schedule an appointment and ask if a virtual visit is right for your care.
  • Does Ascension offer virtual mental health care?

    Yes. If you are struggling emotionally, Ascension Online Care offers virtual mental health services. You can video chat with an experienced psychiatrist, psychologist or counselor online, seven days a week. Whether you are dealing with anxiety, depression, or something else, we're here for you. Just visit the app store on your phone and download Ascension Online Care.
  • How do I find a PCP (primary care provider) in my area?

    To find a PCP in your area, click Find a Doctor. You will be able to browse doctors in your area that are accepting new patients.
  • How do I change my PCP (primary care provider)?

    Changing your PCP is simple. Just find a doctor you are interested in and schedule an appointment. If your doctor is within the Ascension network, your medical records will automatically be available to the new doctor.
