Ascension Personalized Care is only available for residents in the states of Michigan, Indiana, Kansas, Tennessee and Texas. Ascension Personalized Care (also referred to as “APC”) encourages the use of in-network doctors and facilities. Network links are provided as a guide and not a guarantee of in-network coverage. While every effort has been made to provide the most accurate and up-to-date information, network availability can change without notice.

When using the network links on this site, note that:

  •         Networks, in-network doctors, other providers and facilities are not the agents, employees or partners of APC or any of their affiliates or subsidiaries.
  •         APC is not responsible for the content on any network’s webpage.
  •         APC does not provide medical services.
  •         APC neither endorses nor controls the clinical judgment of or treatment by doctors or providers either in- or out-of-network.

In no event will APC be liable to any party for any direct, indirect, special or other consequential damages arising out of any use of this website, or any other hyperlinked website, including, without limitation any losses damages or uncovered charges that may result from receiving care from a doctor, provider or facility listed by any network linked through to our site.

Ascension Personalized Care is committed to protecting your privacy and developing technology that gives you the most powerful and safe online experience. This Statement of Privacy applies to the Ascension Personalized Care site and governs data collection and usage. Any site banner ads link you to areas of interest only within our website. We do not show banner ads for external websites.
